Emergency Notification Card
Children get sick, get hurt, get into trouble, or may just need a ride home. If something happened to your child, wouldn’t you want to know as soon as possible?
Your child’s school needs your correct/updated contact information, including home/work/cell phone numbers, email address, and the name and contact information for anyone who serves as your backup person when you cannot be reached, at the start of school – and each time your contact information changes.
The Emergency Notification Card goes home with every student on their first day of classes and is available in every school building throughout the year. Parents are highly encouraged to complete the form and return it back to the school as soon as possible.
